You may submit completed forms by:
- Scanning and emailing them to the Site Manager (preferred);
- Placing them into the Site Manager drop box located outside the office entrance; or
- Mailing them to the main office address.
When using the drop box, please place the form in a sealed envelope to protect your privacy. Please see the Contact us page for contact information.
The Owner Information Form should be completed by every new owner, and updated when any of the information provided on a previously submitted form changes, including:
- Contact information, including emergency contact information, e.g. mailing address, email address(es), and phone numbers.
- How the unit is used, such as whether part time vs. full time, offered as a rental, etc.
- Occupants (other than renters), i.e. any time an occupant no longer lives at the unit or a new occupant is added.
- Vehicles kept on-site, i.e. any time a new vehicle is added or an old vehicle is removed.
- Whether pets are kept at the unit, i.e. if a new pet is added when there weren’t pets kept beforehand, or if no more pets are kept at the unit anymore. Note that a Pet Registration Form must be submitted for each added pet.
The Tenant Registration Form should be completed by every new tenant, prior to the tenant taking possession of the unit, and updated when any of the information provided on a previously submitted form changes, including:
- Contact information, including emergency contact information, e.g, mailing address, email address(es), and phone numbers.
- Occupants, i.e. any time an occupant, such as a roommate or other resident, moves out of the unit or a new occupant moves in.
- Vehicles kept on-site, i.e. any time a new vehicle is added or an old vehicle is removed.
- Whether pets are kept at the unit, i.e. if a new pet is added when there weren’t pets kept beforehand, or if no more pets are kept at the unit anymore. Note that a Pet Registration Form must be submitted for each added pet.
Tenant Registration Form (PDF)
The Guest Registration Form should be completed for each guest or group of guests visiting at the same time. If you have multiple guests with overlapping stays, but different arrival and/or departure times, please submit a separate form for each guest/group. If any of the information provided on the form happens to change during a guest stay, please submit a new form with the updated information.
The Pet Registration Form should be completed for each pet being kept at a unit. If a pet is only being removed, please submit an updated Owner Information Form or Tenant Registration Form instead.
The required information for Assistance Animal form must be completed for any animal meeting the criteria outlined in the Animal Assistance Policy
